If our nursing assessment indicates that you qualify for home care, we will provide the services ordered by your physician. Our home health staff will get to know you and your family on a personal level. We will work in partnership with your physicians and your community to develop a customized care plan to meet your individual needs.

At Home With A Team Approach

Our home health team consists of health professionals with specialized training in cardiopulmonary care, diabetic care, orthopedic care, wound and skin care, oncology and telemonitoring for hypertension, heart failure, diabetes and respiratory problems.

Our multi-disciplinary team includes:

  • Registered nurses, wound care specialists, infusion specialists and cardiac nurses

Together, our multidisciplinary team can work to provide you with individualized, expert medical care in the comfort of your own home.

Throughout the course of treatment, our home health team will work in partnership with your physician and provide him/her with ongoing updates concerning your progress.

 If there are any significant changes in your health status, your physicians will be contacted immediately to advise us on alternative treatment options.

 Who Is A Candidate For Home Care? & Therapies We Provide

  • Diabetic Patients for Education and /or self treatment instruction (i.e. how to check blood sugar levels and administer insulin injections)
  • Cardiac Patients for teaching and monitoring after acute cardiac events (i.e. heart attack, bypass open heart surgery and chronic hypertension)
  • Dressing changes and treatment of wounds and injuries
  • Full service wound care program with wound care specialists
  • Orthopedic follow-up care or pre-surgical training
  • Physical, speech and occupational therapy required as the result of a stroke, injury or disease
  • Intravenous antibiotic therapy (used to treat different types of infections by administering antibiotics at home)
  • Chemotherapy treatment and follow-up cancer care
  • Medication administration and management
